My plan for life guide

The Fit for Work UK Coalition was part of an alliance of organisations that recently launched the ‘My plan for my life with…’ guide – a care coordinating guide. The aim of 'My plan for my life with…' is to help people with chronic musculoskeletal conditions to think about what matters most to them and what they want from their care – including returning to work. The message to patients is: you are the expert on how your condition makes you feel. If you can think about what is important to you, you will be able to ask the right questions and get the right information and support.

Patients can use the guide to note down questions that they want to ask and any advice they've received. They can also use it to record the contact details for the different members of their healthcare team in one place. It has space to record appointments, test results and discussions too.

The guide has been created by a coalition of organisations working to improve care for people with musculoskeletal conditions. In developing it, they tested the content and design with real people living with or supporting people with musculoskeletal conditions. The project was facilitated by the Fit for Work UK Coalition and the following organisations participated in the development of the guide: Arthritis and Musculoskeletal Alliance, Arthritis Care, BackCare, British Society for Rheumatology, Chartered Society of Physiotherapy, College of Medicine, National Ankylosing Spondylitis Society, National Rheumatoid Arthritis Society, Neil Betteridge Associates, Patient Information Forum, Royal College of General Practitioners, and The Work Foundation.

The guide is being piloted in different regions of the UK. Patients are providing feedback through an anonymous questionnaire. Results from the pilot evaluation will be available by the end of 2016.
